Definition of "Gonorrhea"

Gonorrhea (aka the clap, gonococcal infection) is a bacterial STI.

Pathophysiology
  • Caused by N. gonorrhoeae
Sx
  • In men:
    • Dysuria
    • Penile discharge
  • In women:
    • Can be asymptomatic (50%)
    • Vaginal discharge
    • Pelvic pain
Ix
  • PCR
  • Traditionally, Dx with gram stain and culture. However, culture can still be done to determine sensitivity to abx
  • Should also be tested for other STI's, e.g. chlamydia, syphilis, and HIV
Tx
  • Ceftriaxone (Rocephin), as antibiotic resistance has developed to many previously used medications
  • Chlamydia medications may be given in combo as ceftriaxone won't cover
Complications
  • Left untreated, like chlamydia, can cause:
    • PID in women
    • Epididymitis in men
  • Throughout the body, affecting joints and heart valves
