0: Healthy periodontium (PD<3.5mm)
1: Bleeding/ Plaque present (PD<3.5mm)
2: Calculus detected (PD<3.5mm)
3= Probing depth 4-5mm
4= Probing depths 6mm


• PSR 0-2 and SPT are stable perio conditions
• PSR 3-4 in 2+ sextants, is a "periodontitis" Pt, so requires full perio exam, inc comprehensive risk assessment and full charting
• Generalised PSR 2 and maximum of 2x sextants w/ score 3 (inc SPT Pt's w/ stable perio conditions) is Yr 4
• Up to maximum of 3x sextants w/ PSR 3 (or 2 w/ PSR 3 and 1 sextant w/ PSR 4) is Yr 4 or 5
• >3 sextants w/ PSR 3/4 and all other combinations not included above will be referred to PG
