• PSR 0-2 and SPT are stable perio conditions
• PSR 3-4 in 2+ sextants, is a "periodontitis" Pt, so requires full perio exam, inc comprehensive risk assessment and full charting
• Generalised PSR 2 and maximum of 2x sextants w/ score 3 (inc SPT Pt's w/ stable perio conditions) is Yr 4
• Up to maximum of 3x sextants w/ PSR 3 (or 2 w/ PSR 3 and 1 sextant w/ PSR 4) is Yr 4 or 5
• >3 sextants w/ PSR 3/4 and all other combinations not included above will be referred to PG

Caries experience (caries prevalence): (0-3)
0= Caries free and no fillings
1= Better than normal
2= Normal for age group
3= Worse than normal

Related (general) disease: (0-2)
0= No disease
1= Disease/conditions, mild degree
2= Severe degree, long-lasting

Diet, contents: (0-3)
0= Very low fermentable carbohydrate
1= Low fermentable carbohydrate, "non-cariogenic" diet
2= Moderate fermentable carbohydrate content
3= High fermentable carbohydrate intake inappropriate diet

Diet, frequency: (0-3)
0= Maximum 3 meals per day (including snacks)
1= Maximum 5 meals per day
2= Maximum 7 meals per day
3= More than 7 meals per day

Clin judgment: (0-3)
0= More positive than what the Cariogram shows based on the scores entered
1= Normal setting! Risk according to the other values entered
2= Worse than what the Cariogram shows based on the scores entered
3= Very high caries risk, examiner is convinced that caries will develop, irrespective of what the Cariogram shows based on the scores entered

High risk: <50%
Moderate risk: 50-74%
Low risk: 75-100%

Dark blue sector (Diet): Combination of Diet contents and Diet frequency
Red sector (Bacteria): Combination of Amount of biofilm and Mutans streptococci
Light blue sector (Susceptiblity): Combination of Fluoride program, Saliva secretion, Saliva buffer capacity
Yellow sector (Circumstances): Combination of Past caries experience, Related disease
Green sector (Estimation of actual chance to avoid new cavities): the bigger this sector the better from a dental point of view
