Definition of "LGA"

Large for gestational age (LGA) is an indicator of high prenatal growth rate.

Risk factors
  • Poorly controlled maternal diabetes, a major risk factor, especially gestational diabetes, as well as pre-existing type 2 diabetes. [Pre-existing type 1 diabetes is associated with microsomia] This is because increased maternal blood glucose stimulates fetal growth. This causes macrosomia (i.e. affecting weight only)
Dx
  • Weight, length, or head circumference> 90th percentile for that gestational age
    • It has been suggested that the definition be further restricted, to infants with birth weights >97th percentile (2 standard deviations above the mean)
  • Macrosomia (meaning "big body" is where WEIGHT alone> 90th percentile, which is >4kg at birth
Tx
  • Induction (16% of all inductions)
  • C-section (9% of all C-sections)
Prognosis
  • Usually good in a majority of cases
